Feel alive and face each day head-on knowing that you have an amazing team like Altitude Care by your side. We will match you with a friendly support team who will assist you in your home with household tasks, personal hygiene, meals, toileting and mobility around the house. In-home care is there to support anyone who needs regular care in the home and high intensity needs like overnight care. Our carers are fully trained in this area of disability support and will even help you with your daily medication, hoists or wheelchair mobility.
Living in a clean, tidy environment is paramount to your health and wellbeing. We offer professional cleaning services for your home and garden so you can enjoy a beautiful, welcoming sanctuary each day. Altitude Care will provide expert cleaning and gardening services to help you with things like mowing the lawn, weeding, planting new blooms, or cleaning floors, bathrooms, kitchens, dusting, polishing and more. This is separate to the services provided by your in-home support worker, so you can go on receiving quality care while your home is cleaned by the professionals. We help to make your time spent at home or in the garden more pleasant with neat, inviting spaces and friendly service.
At Altitude Care we reach for the stars and we want the same for you. We support you in a way that builds your capacity to live more independently. Our Capacity Building service includes anything that will help you develop life-long skills to manage tasks on your own. This could be learning how to make your own nutritious meals, taking public transport on your own, managing your finances, managing your NDIS supports or doing your own grocery shopping. We help you find your strengths and build on them to become more capable and confident.
Community is what helps us connect as human beings. At Altitude Care we believe that community connections are vital to your self-development and everyone deserves the right to community access. With this support you can participate in events and social or sporting activities and make friends, improve your independence and find like-minded people to add to your circle of support. When you have access to activities such as swimming, art, library trips, cooking, volunteering or day trips, you not only get to do fun things that interest you, but make great friends in the process.
Our friendly community nurses have a positive attitude to clinical care and will make sure you stay in good health. We provide professional nursing services in the comfort of your home and can assist with things like medication administration, wound care, catheter feeding, diabetes management and more. If having a nurse with you 24/7 is something you require for safety and peace of mind, you can rely on our team to deliver respectful, reliable nursing round the clock. The difference with Altitude Care is that our nurses see you as a person and not just a patient. We truly love what we do and it shows in our demeanour and personalised approach to care.
We offer Support Coordination Level 2 which involves hands-on coordination of your supports, helping you to gain better control over your life and not just your services. We work together with you to help you understand your funding so you know exactly what to expect from your supports, and we guide you in designing how you want your supports to be delivered for your personal benefit.
A psychosocial recovery coach plays the same role as a support coordinator but has specific knowledge in mental health and can help participants with mental illness to coordinate their supports with a focus on psychosocial needs. Our recovery coaches are highly experienced and qualified to provide you with targeted support coordination that is tailored for your mental health needs if you have a primary diagnosis of psychosocial disability.
